Every participant in our programs, including family members and personal care assistants, will need to fill out a liability waiver. Please download appropriate form and bring your completed form to the program. You just need to fill out the form
one time per calendar year.
Programs run in partnership with the Universal Access Program require a UAP Liability Waiver. These are all of our programs on Department of Conservation and Recreation properties, including the Norwottuck and Ashuwillticook Rail Trails, DAR and Mt. Tom State Forests, Hopkinton, Lake Quinsigamond, Blackstone and Worcester cycling, and indoor Ice Skating programs.
